Walk or Mail In Registration
Use the forms below to register your family for recreation programs through mail-in or walk-in. Click on the form title to print off a PDF of the form. Fill it out and mail it in or stop by with it and we will help you. If you mail in any form you will receive an email of your receipt for the programs you have registered for.
Some programs are not available for on-line registration or they have special registration forms.
Household Information Form
This form needs to be filled out only once for your household to be placed in our database. It will enable you to register by mail, online, or at the office. This form includes the Mail-In Registration Form also.
Mail-In Registration Form
This form is to be used for mail-in registrations once you are in our system. If you are not fill out the above Household Information Form along with this form and send both with full payment. If you are in our system, you only need to fill out this form and mail in with full payment.
